Scheduler / Patient Care Coordinator

We're currently seeking highly organized and proactive professionals to join our growing network of Scheduling, Care Coordination, and Patient Support opportunities.

We work with multiple clients within the Healthcare and Home Care industries, offering a variety of opportunities depending on business needs. Positions may vary in work arrangement, schedule, and level of responsibility.

These roles are ideal for candidates with experience coordinating patient services, managing schedules, supporting caregivers, handling care coordination activities, and maintaining strong relationships with patients, clients, and healthcare teams.

Key Responsibilities

• Coordinate appointments, schedules, and service coverage.
• Support patients, caregivers, and clients with scheduling and care-related needs.
• Manage schedule changes and coverage requests.
• Maintain accurate records, documentation, and system updates.
• Communicate effectively with patients, caregivers, providers, and internal teams.
• Assist with care coordination and service delivery activities.
• Resolve scheduling issues and operational challenges.
• Ensure continuity of services through proactive planning and follow-up.
• Support administrative and coordination processes as needed.

Requirements

• Previous experience in Scheduling, Patient Coordination, Care Coordination, Client Care, Medical Scheduling, Home Care Operations, Healthcare Administration, Intake Coordination, Case Management, or similar roles.
• Strong organizational and multitasking skills.
• Excellent verbal and written communication skills.
• Ability to work effectively in fast-paced environments.
• Experience using scheduling software, EMR/EHR systems, CRM platforms, or similar tools.
• Proficiency with Microsoft Office, particularly Excel and Outlook.
• Experience in Healthcare, Home Care, Medical Offices, Clinics, Hospitals, or Patient Services is strongly preferred.
• Advanced English communication skills preferred.
