Lenawee Community Mental Health Authority of Adrian MI is looking to hire a full-time Case Manager - Home Based (Children's Services). This position is onsite and requires travel to/from the consumer's location. This position will provide intensiveservices to children and their families, who have multiple service needs and access to an array of mental health services. The Case Manager - Home Based is responsible for supporting families in meeting their child's developmental needs, providing support to preserve families, to reunite families who have been separated, and to provide effective treatment and community supports based on the child's needs, with the focus on the family unit. The starting pay for this position is $1,038.00 per week.

A Day in the Life of a Case Manager - Home-Based
You assist eligible minor consumers (and their families) with the development of a youth-driven / family guided Person / Family-Centered Plan (P/FCP) emphasizing strength-based, culturally relevant interventions, parent/youth and professional teamwork, and connection with community resources and supports. You work with the minor consumer (and their families or chosen support network) in developing a P/FCP that promotes individual choice, the use of community resources, and formal and informal supports in pursuit of the minor consumer's desired goals. You effectively monitor the supports and services according to the plan. You will find creating a path to resilience, recovery, wellness, and self-determination for the youth that you work with to be very rewarding!
