This is an ideal role for an APP who enjoys hands-on patient care, clinical independence, and interdisciplinary collaboration without the burnout of traditional acute-care settings.

What You'll Do

  • Conduct comprehensive in-home patient assessments
  • Perform full physical exams and clinical evaluations
  • Manage medications and individualized care plans
  • Provide chronic disease education and coaching
  • Complete hospital transition and follow-up visits
  • Collaborate in interdisciplinary team rounds reviewing high-risk patients
  • Document in a fully integrated EMR (Athena) with real-time care team access
Patient Volume
  • 4-5 patients per day
  • New patient visits: ~90 minutes
  • Follow-up visits: 30-60 minutes
Schedule & Flexibility
  • Hybrid model
    • 3-4 days: in-home patient visits
    • 1-2 days: remote admin, charting, and telehealth
  • Monday-Friday schedule
  • No evenings, weekends, or on-call responsibilities
  • AI-assisted routing for optimized travel efficiency
Compensation & Benefits
  • $135,000 - $150,000 base salary (experience dependent)

Qualifications
  • Nurse Practitioner or Physician Assistant license (NJ)
  • Minimum 2 years APP experience
  • Experience managing longitudinal care plans preferred
  • Strong patient education and relationship-building skills
  • Comfort working independently in community settings
