Amivie - - Responsibilities: Assist the individuals with goal completion as documented in the individualized support plan; Provide assistance with Activities of Daily Living (ADLs) based on individual needs/person centered plan; Safely transport Individual(s) in company or personally owned vehicles; Accompany the Individual(s) as needed to community integration activities; Complete all required documentation in the electronic health record (EHR) timely
