Amivie - - Responsibilities: Assist the individuals with goal completion as documented in the individualized support plan.; Provide assistance with Activities of Daily Living (ADLs); (i.e. bathing, grooming, brushing teeth, toileting, feeding, preparing meals, etc.) based on individual needs/person centered plan.; Safely transport Individual(s) in company or personally owned vehicles.; Complete all required documentation in the electronic health record (EHR) timely.; Actively engage with Individual(s) in their home and community
