Schedule: PRN (as-needed) position. Team members are required to pick up a minimum of 15-20 shifts per month.
Available shift options include:
Weekdays: 5:00 PM - 8:30 AM
Weekends: 8:30 AM - 5:00 PM (day) 5:00 PM - 8:30 AM (overnight)
Flexibility to work a mix of weekday and weekend shifts is expected.
Rate: $35/hour
Benefits: 401(k) with company match (100% match on the first 3%, 50% match on the next 1%)

What you will do
- Provide on-call psychosocial support to hospice patients, caregivers, and families by phone and in person.
- Respond promptly during assigned on-call shifts and review reports, emails, and messages at shift start.
- Provide crisis intervention, grief support, education, and counseling per the hospice plan of care.
- Support admissions, changes in level of care, and end-of-life needs in collaboration with the after-hours care team.
- Complete accurate, timely documentation and shift reports per agency standards.
- Master's degree in social work (from a school of social work accredited by the Council of Social Work Education); LCSW or licensure eligible preferred;
- At least three years of social work or counseling experience; hospice or end-of-life care preferred.
- Strong skills in crisis intervention, grief support, case management, and critical thinking.
- Excellent communication, collaboration, and ability to work independently.
- Compassionate, emotionally mature, and mission-driven with a heart for end-of-life care.
