Master of Social Work, PRN

Join James River Home Health, where your compassion and expertise help patients thrive at home. We're looking for a PRN Home Health and Hospice Social Worker to provide vital emotional, social, and resource support to patients and families as part of our collaborative care team.

Schedule: Flexible PRN Schedule
Areas of Coverage: Roanoke and Surrounding Areas

A brief overview of Responsibilities:

• Complete psychosocial assessments and participate in patient admissions
• Develop and implement individualized care plans
• Provide short-term counseling, crisis intervention, and discharge planning
• Help patients access community resources and navigate challenges
• Document visits and collaborate with physicians and clinical staff
• Supervise Social Work Assistant monthly
• Participate in team meetings, in-services, and ongoing education

Qualifications:

• Master's Degree in Social Work (MSW) from an accredited program preferred
• Certification from the Academy of Certified Social Workers (preferred or in progress)
• At least one year of experience in a healthcare setting; two years preferred
• CPR certification and valid driver's license required
• Criminal background check required
