About the Role

Our rapidly expanding, multi-site healthcare organization is searching for a highly skilled Accounts Receivable & Collections Manager to build, lead, and scale a best-in-class AR and denial prevention function. This is a role for someone who thrives in fast-growth environments, loves solving complex AR challenges, and has a proven track record of recovering legacy and extremely aged AR across diverse payer types.

Key Responsibilities

AR & Collections Leadership

  • Build a comprehensive AR and collections department from scratch, including workflows, staffing models, KPIs, and reporting structures.
  • Lead the recovery of legacy AR, including accounts aged 180+ days, with a focus on high-complexity payer scenarios.
  • Develop and implement aggressive yet compliant collection strategies tailored to commercial, government, and workers' compensation payers.
  • Establish and maintain AR dashboards, aging reports, and performance metrics to drive accountability and transparency.
Denial Prevention & Revenue Optimization

  • Analyze denial trends and root causes; collaborate with coding, billing, and clinical teams to implement prevention strategies.
  • Create standardized denial management workflows and escalation pathways.
  • Partner with payers to resolve systemic issues and negotiate resolutions for high-value or disputed claims.
  • Ensure compliance with California-specific billing, reimbursement, and workers' compensation regulations.
Operational Excellence

  • Design and refine end-to-end AR processes to support a multi-site, high-volume practice.
  • Train and mentor staff on best practices in collections, denial management, and payer communication.
  • Collaborate with finance leadership to forecast cash flow, identify revenue risks, and support month-end close activities.
  • Implement technology solutions (EHR, clearinghouse tools, analytics platforms) to streamline AR operations.
Qualifications

Required
  • 5+ years of progressive experience in healthcare AR, collections, and denial management.
  • Demonstrated success recovering legacy and very old AR, including complex payer scenarios.
  • California healthcare billing and reimbursement experience (non-negotiable).
  • Workers' compensation expertise, including knowledge of CA WCAB rules, fee schedules, and dispute processes.
  • Proven ability to build or rebuild AR/collections teams, systems, and workflows from the ground up.
  • Strong understanding of payer contracts, appeals, and regulatory requirements.
  • Experience in multi-site or multi-specialty healthcare organizations.
Preferred

  • Experience in a fast-growth or private-equity-backed healthcare environment.
  • Familiarity with high-volume outpatient or specialty practices.
  • Advanced Excel or analytics skills.
What Success Looks Like
  • Rapid reduction in aged AR, especially 120+ and 180+ day buckets.
  • Significant improvement in denial rates and first-pass resolution.
  • Strong cross-departmental collaboration and payer relationship management.
  • Predictable, optimized cash flow supporting organizational growth.
