Seeking Cardiovascular experience nurse to join Advanced Heart Failure Team located at Cardiovascular Institute in Crystal Spring Tower as a Heart Failure Clinical Coordinator supporting patients with advanced heart failure/ LVA - Ds using advanced nursing processes in decision-making, patient assessment, data analysis, development and implementation of methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. Provides direct and indirect patient care as needed; oversees the delivery of patient care; and provides day to day management of heart failure programs (Infusion, LVAD, HF remote monitoring) to coordinate patient care as designated by the department. Coordinates the delivery of safe, high quality patient care; identifies areas for study; and participates in and/or supports research.
