Sentara Health Plansis hiring a Temporary Care CoordinatorIn the Roanoke Region Virginia!
area includes: Roanoke, Lexington, Buena Vista, Daleville, Buchanan, Covington, New Castle, Bedford, Rocky Mount, Martinsville, Stuart, Blacksburg, Radford, Wytheville and surrounding areas!
This position has full time hours and will last through the end of June 2023 with the potential of extending or going permanent. The position requires both in person face-to-face assessments and remote telephonic assessments!
Applicants must be located in Roanoke Virginia Region
As a Temporary Care Coordinator you will perform a variety of casework duties and provide case management services to patients, families, and designated caregivers. You will help develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical conditions up to and including at time of terminal illnesses.
Minimum Qualifications:
- 4-year degree in Health and Human Services
- Long Term Care/Medicaid/Medicareexperiencerequired
- Experience with Community Health Resources strongly preferred
- Experience initiating care plans and both subjectively and objectively conducting assessments
- Experience with educating members with chronic disease(es) strongly preferred
- Mental Health experience strongly preferred
